Pre-requisite qualifications
- Ability to study at level M (normally has obtained 120 credits at level 3)- A pharmacist with current registration with the GPhC or Pharmaceutical Society of Northern Ireland (GPhC)
- Access to appropriate pharmacy practice to allow the undertaking of work-based assignments
- Appointment of an agreed work-based tutor and support from a work-based manager.

Objectives
On completion of this module, students should be able to:- critically analyse the pharmacotherapy of core conditions in relation to national and local guidelines and the patient's individual needs;
- design and evaluate pharmaceutical care plans taking into account the need for continuity of care;
- demonstrate an in-depth knowledge of the aetiology and clinical features of a range of core conditions
- critically evaluate key factors influencing the adherence of patients to their medication in relation to patients with core conditions;
- take a proactive and self reflective approach to the pharmacist's role in the patient care in the context of core conditions to inform CPD.
Learning outcomes
Aetiology and clinical features of range of core conditions:
- Therapeutics of a range of core conditions including key national guidelines and supporting evidence
- The pharmacists role in care of core conditions
- Factors affecting adherence to medicines of core conditions.
Skills outcomes
To be able to provide pharmaceutical care to a range of patients with core conditions.
Syllabus
- Planning, managing, monitoring and reviewing pharmaceutical care plans
- Understanding and adopting evidence-based medicine/practice
- Aetiology, clinical features and critical evaluation of pharmacotherapy for a range of core conditions
- Principles of adherence and concordance
- Reflection on the pharmacist's role and CPD requirements.
Core conditions:
- Cardiovascular: Heart failure, Angina, ACS, MI, Atrial Fibrillation, PE, DVT, Thromboprophylaxis
- Gastroenterology: Dyspepsia, GORD and Peptic Ulcer disease
- Respiratory: COPD and Asthma
- Infection: Pneumonia, Cellulitis, UTI and pyelonephritis
- Endocrine: Diabetes and Osteoporosis
- Mental Health: Depression and Dementia
- Musculoskeletal: OA and Gout
- Surgical patient: Management of long term medicines, fluid balance, infection, PONV and acute pain
- Care of the Older person.
