Module summary
Case complexity can arise for multiple reasons including psychological factors (e.g. phobias), medical complications, multi-morbidities or an accumulation of multiple dental challenges. Integration of expertise from the relevant disciplines can deliver substantial patient benefits, enabling successful oral health outcomes. This module presents multiple examples illustrating a step-by-step approach to the integration of care across multiple disciplines for such cases. Objectives
The aims of this module are:• To elucidate the concept of clinical complexity and its many elements and manifestations
• To discuss and agree a protocol for the management of the complex case
• To use best evidence to develop patient-centred skills in the management of multi-factorial cases, including when and how to implement an integrated team approach
• To consider the prioritisation of treatment need and creation of effective treatment plans
Learning outcomes
1. Evaluate the nature of the complexity that may arise from procedural, oral/general health morbidities, polypharmacy or psychosocial problems and other factors that make oral health care challenging
2. Explain, in-depth, the techniques used to identify, recognise and manage multiple morbidities among patients presenting for oral care (this includes eliciting patients’ concerns and priorities)
3. Discuss the mechanisms of the two-way relationship between general health morbidity, polypharmacy or psychosocial problems and perceived oral health
4. Synthesise patient-centred oral care plans that address issues of such complexity
5. Judge when and how to work in conjunction with other health care professionals
Skills outcomes
The understanding gained in this module will strengthen clinical skill and aptitude
Syllabus
• Recognising and identifying psychological, physical and procedural challenges of complex clinical cases
• Dissection and analysis of the component parts of a complex case
• Diagnosis of pain
• Reflection on one’s own personal capacity to deal with complexity
• Working as part of an integrated team
• Use of a series of simulated cases that facilitate discussion and understanding of the various elements of integrated oral health care provision.
• Topics covered will include chronic pain, psycho-social factors and phobias, medical implications and polypharmacy, co-morbidities and the presentation of multiple dental challenges
