Pre-requisite qualifications
- Ability to study at level M ( normally has obtained 120 credits at level 3)- A pharmacist with current registration with the GPhC or Pharmaceutical Society of Northern Ireland (GPhC)
- Access to appropriate pharmacy practice to allow the undertaking of work-based assignments
- Appointment of an agreed work-based tutor and support from a work-based manager.

Objectives
On completion of this module, students should be able to:- demonstrate an in-depth knowledge of the aetiology and clinical feature of a range of specialist conditions;
- critically analyse the pharmacotherapy of these conditions in relation to national and local guidelines and the patient's individual needs;
- critically evaluate pharmaceutical care plans taking into account the need for continuity of care;
- critically evaluate key factors influencing the adherence of patients to their medication in relation to patients with specialist conditions;
- take a proactive and self reflective approach to the pharmacist's role in the patient care in the context of acute conditions to inform CPD.
Learning outcomes
Aetiology and clinical feature of a range of specialist conditions:
- Therapeutics of a range of specialist conditions including key national guidelines and supporting evidence;
- The pharmacists role in care of patient with specialist conditions;
- Factors affecting adherence to medicines in relation to patient with specialist conditions.
Skills outcomes
To be able to provide pharmaceutical care to a range of patients with specialist conditions.
Syllabus
- Planning, managing, monitoring and reviewing pharmaceutical care plans
- Understanding and adopting evidence-based medicine/practice
- Aetiology, clinical features and critical evaluation of pharmacotherapy for a range of core conditions
- Adherence and concordance as related to specialist conditions
- Reflection on the pharmacist's role and CPD requirements
- Principles of pharmaceutical care of paediatric patients.
Specialist conditions:
- Dermatology: Psoriasis, Eczema, Acne
- Gastroenterology: Inflammatory bowel disease, Irritable bowel syndrome
- Mental Health: Acute Psychosis, Schizophrenia, Bipolar disease
- Musculoskeletal: Rheumatoid Arthritis
- Neurology: Epilepsy, Parkinson’s disease, Stroke
- Infection: Osteomyelitis, TB, meningitis, endocarditis, neutropenic sepsis
- Endocrine: Hypo/Hyperthyroidism
- Critical Care: Shock
- Palliative care (including neuropathic pain)
- Chronic renal failure
- Liver failure
- Substance misuse.
