Module summary

This module will provide learners with knowledge and understanding of how to care for a person with a long term condition.

Objectives

This module will provide learners with integrated knowledge and understanding of the pathophysiological, psychological and sociological impact that long term disorders have on the individual and their family, to enable assessment and nursing care to be applied and underpinned by relevant nursing knowledge as well as policy and research. Students will explore the role of the nurse, multi-professional care and team working with service users and carers with long term conditions. The effectiveness of self- management strategies and service user/carer partnerships, in relation to a variety of long term conditions will be considered. Issues related to the person's safety within the hospital /or home environment and the need for risk assessment will be addressed.

Learning outcomes
On completion of this module students will:
- Know the rationale for undertaking a comprehensive nursing assessment of the person experiencing a long term condition
- Understand in detail the nursing role in caring for a person experiencing a long term condition.
- Be able to describe the pathophysiology of a variety of long term conditions e.g. neurological, cardiac, diabetes
- Appreciate the disease progression indicators (acute/chronic/relapse /end of life) with regard to long term conditions
- Have appraised the effectiveness of self management strategies and service user/carer partnerships, in relation to a variety of long term conditions.
- Have compared and contrasted the challenges to the persons safety within the hospital /or home environment in regard to risk assessment
- Be able to analyse the nature and role of multidisciplinary team and inter professional relationships with respect to the management and care of people with a long term condition
- Understand the concept of quality of life and analyse how the individuals QOL is altered by living with a long term condition
- Have considered end of life issues for people with a long term condition
- Have explored the research evidence which underpins current and national guidelines for people with a long term condition.
Have an understanding of the epidemiological and demographic issues, diversity, inequality in relation to access and suitability of long term care services
- Consider long term care issues for an ageing population

Skills outcomes
Students will consolidate skills in relation to the nursing process in relation to patients with long term conditions. The concept of Quality of life will be developed and consideration of how that concept applies to this group of patients. Students will further develop skills of accessing and evaluating epidemiological data in relation to this group of patients and of service development. Skills of supporting self-management of care and medication will also be developed.

Psychological theory/sociological theory in relation to adaptation, role transition/adjustment/body image self- esteem, self -concept and how these perspectives alter across the lifespan and as the long term condition develops
Nursing theory related to self-care, adaptation
Specific nursing/patient issues; communication, nutrition, continence, Risk assessment in relation to falls and cognition.
Ongoing care issues
Rehabilitation
Pain and its management/ loss and grief, end of life issues, palliative care
Sexuality/spirituality
Mental health issues, depression, dementia
Mobility and the prevention of physical problems
Multidisciplinary and Inter-professional working

Private study

Independent learning is through a variety of on line articulate presentations and reading that the students have time allocated to read and prepare for the taught sessions. These relate to long term conditions as well as associated issues that the person with a long term condition may experience.

Opportunities for Formative Feedback

- Over the course of the module, feedback during classroom activities will help students understand the complexities of delivering care to individuals living with long term conditions.
- Academic support will be in accordance with the School of Healthcare recommendations.


Monitoring of Progress:
Students will be monitored through their ability to apply information within the tutorial groups as well as through completion of the online learning.
