Module summary

This is an introductory course for students on the Dentistry or Medicine Integrated International Foundation Year which will provide these international students with experience of healthcare and voluntary sector care provision within the UK. By providing students with experience of care in both clinical and voluntary care settings, students taking this module will become more familiar with how the health system works in the UK. In addition, students will develop their awareness of diversity and health-related English language skills, through exposure to the lived experiences of patients from a variety of backgrounds. Students will also improve their independent learning strategies through experiencing case-based learning. The course has been designed for International Foundation Year students intending to proceed to Dental Surgery or Medicine at the University of Leeds, with a view to providing these students with comparable learning experiences to those of UCAS entry students.

Objectives

The aims of this module are for students to:
- gain observational experience of patient care.
- develop their communication skills.
- develop independent study strategies.

Learning outcomes
Learning outcomes:
By the end of this module students will:

1. have an enhanced ability to learn from and reflect on their experiences in clinical environments and voluntary care settings.
2. have developed communication skills in both informal and formal settings with: patients, simulated patients, health care professionals, students and academics.
3. have developed study strategies that will enable future independent study of Basic and Clinical Sciences.
4. have an understanding of social inequality, and will be able to demonstrate empathy when interacting with vulnerable people with a diverse range of needs.
5. have an understanding of the NHS, and of the regulation of UK healthcare by the GDC or GMC.
6. be able to recognize the role of the dentist or doctor as part of a team of healthcare professionals, but also be able to recognize the importance of exploring the patient and carer perspective in health.
7. be more familiar with the Dental / Medical programmes of study at the University of Leeds.

Skills outcomes
By the end of the module, students should be better able to:
- understand the provision of healthcare within the UK and reflect on the interactions that they have seen
- understand the diversity of patients and be more aware of social inequalities in healthcare.
- communicate with patients, simulated patients, health care professionals, students and academics
- demonstrate the application of independent study strategies

Private study

Private study will include the reading of support material specifically written for the module available on the VLE. Key texts and websites will be signposted to students to assist them in: reflecting on what they have observed, preparing for practical sessions, and revising for assessments. Specific activities include:
- Further reading prior to and after small group taught sessions.
- Reading related to clinical observations and interacting with patients.
- Arranging and preparing for the voluntary placement.
- Preparing for the observed SP interaction.
- Revising for the exam.
- Conducting research for the case based learning sessions

Opportunities for Formative Feedback

This module is continuously assessed. There is a constant flow of informal feedback to students through the communicative interactions they are engaged in in clinical and voluntary settings. Students receive ongoing informal feedback from current dental / medical students and members of the patient carer community, in addition to feedback from the tutor regarding their communicative competence. Students also receive informal feedback on drafts of their portfolio reflections.
In terms of summative assessment, it is proposed that students are directly assessed on their communicative competence in Semester 2 in an assessed Simulated Patient (SP) interaction counting 25% of the module grade. The students will have had a lot of speaking practice prior to this interaction through all the informal opportunities they have had to practice speaking with patients, dental / medical students, dental / medical practitioners and in the SP roleplay session in Semester 1. Being able to speak English in a health context is a key aim of this module. At the moment communicative competence is captured only indirectly in portfolio reflections on communications domains. Students currently present their portfolios in Semester 2 in a summative assessment counting 50% of the module grade.
It is proposed that this portfolio is in future used to assess students ability to learn from clinical observation only (25% of the grade), with the communications domain being taken out and used as a basis from which to develop a criterion based rubric for assessing the SP interactions which will count 25% of the module grade.
Students’ understanding of the NHS, roles of the dental / medical team, basic dental and medical health issues, and their understanding of the basic Science underpinning clinical scenarios is assessed in the final exam (50%), and this will remain unchanged.

If students are not present for the assessed SP interaction, then they will be given another opportunity to complete this at a later date, similarly with unsuccessful students resitting. If students are not successful in completing the portfolio (progressional element), they will be given a written task of equivalent length to complete. If students are not successful in writing the exam, they will be given a July resit opportunity.

A July resit will be available for the exam. The International Foundation Year team has asked that resits be done in July (not August) to give students time to arrange VISA’s etc. before the start of the new academic year. The three assessed components (Exam, SP interaction and portfolio) will be compensatable. OSCE - The format of this assessment may be subject to change as a result of restrictions imposed as a result of the Covid-19 epidemic but the learning outcomes will remain unchanged and approval of the appropriate PSRB (Professional, Statutory and Regulatory Bodies) will be obtained. Online time limited assessment with a duration of 48 hours.
