Module summary

This module, the fourth in the ‘Clinical Practice’ modules thread, gives the opportunity for further clinical experience in a range of different speciality areas and in a range of clinical dental settings. The module runs throughout the entirety of year 4. The majority of the time will be spent providing care for patients within the Leeds Dental Institute (Restorative Dentistry, Oral Surgery, Acute Dental Care, Radiology and Paediatric Dentistry). You will also gain experience of working in a range of clinical dental settings outside including outreach clinics, orthodontic practices and general hospitals. You will gain more experience of working with assistants and be given the opportunity to assist for others. You will be able to work with electronic patient record systems. You will be expected to comply with all cross infection, health and safety and clinical governance protocols that are necessary for safe practice within the LDI as well as observing the relevant local practices in other settings. You will develop your ability to become a reflective practitioner and become aware of areas that need improvement, be well prepared for the procedures you are being expected to carry out and be proactive in seeking guidance and further development. You should be able to use tutor, peer and patient feedback effectively to reflect on your progress and to plan your ongoing development. You will also be able to provide feedback to your peers and colleagues. You should be able to embrace the ideals of evidence-based dentistry and begin to take ownership of your own lifelong learning and continued professional development. Feedback will be provided by clinical tutors and progress will be monitored by the clinical progress committee. You will continue to use the CAFS system to record your clinical experience, feedback and reflection. Completion of this module is necessary to progress to ‘Clinical Practice 5’, the culmination of the ‘Clinical Practice’ theme.*Students will be permitted to undertake certain areas of clinical operative management following completion of relevant clinical skills courses and clinical gateways. Students are not expected to have demonstrated these areas of clinical operative management by the end of year 4. There is further opportunity to undertake these areas in year 5 of the programme.Unless your programme is designed to be delivered fully online, your programme and modules will be delivered predominantly on campus. These teaching activities will be supported by the use of digital tools to enhance your learning and experience. You will be kept informed of any proposed changes to the content and delivery of your modules as described in the module catalogue and we’ll make sure you understand and agree the reasons for any changes before they are introduced.

Objectives

This is the 4th module in the five-year clinical practice thread. In this module students will further develop their ability to take direct responsibility for the clinical management of dental patients.
The module objectives comprise the themes of:
- Assessment of patients
- Clinical operative management
- Prevention
- Clinical Professionalism
- Reflection
These themes are developed throughout the programme within the Clinical Practice (ICSP/CP 2-5) modules.
In this CP4 module students will observe and treat patients within the clinics of the Leeds Dental Institute and in outreach centres, general hospitals and specialist dental practices.
The module provides experience of working in real-world clinical settings, such as community health clinics, maxillofacial departments and orthodontic practices.
Students will be encouraged and expected to continue to develop the knowledge and skills required to perform the safe, reflective and independent practise of dentistry. They should be able to provide a holistic dental treatment plan for adult and child patients who require dental care to a non-specialist level. They should be able to provide simple, non-surgical extractions, local anaesthetic procedures, plastic restorations, non-surgical periodontal treatment and preventive care within real life clinical environment.
The module will encourage development of a level of professionalism commensurate with the increased level of responsibility that comes with being a senior clinical student. This will involve teamworking, communication and management and leadership including the ability to assist other clinicians, liaising with other specialists and being aware of their place in the dental team.
The module will develop awareness of how the dental surgeon contributes to holistic, excellent patient care, as well as ensuring collaboration with other health care workers to achieve the best outcomes for the patient.
By the end of the module, students will be expected to have demonstrated their ability across all learning outcomes through a range of assessments:
- Formative (continued clinical assessment and feedback recorded in CAFS)
- Progressional (Clinical Gateway assessments, OSCE)
- Summative (Periodontal case report and a written examination)
The clinical progress of students will be monitored throughout the module by the School of Dentistry Clinical Progress Committee.

Learning outcomes
On completion of this module the students will be able to:
1. Assessment of patients:
Carry out a comprehensive patient assessment and assimilate clinical information to establish appropriate diagnoses and devise an evidence based treatment plan in the context of whole patient care in a child or adult patient under close supervision.
2. Clinical operative management:
Apply the principles of evidence based dentistry to the provision of routine dental procedures for managing common dental and oral conditions in uncomplicated clinical scenarios under close supervision.
3. Prevention:
Identify oral and general disease risk factors, plan and manage a course of tailored, holistic oral and general health advice and preventive care in adult and child patients with increasing independence.
4. Clinical Professionalism:
Practise professionalism independently including team-working, communication, ethics and leadership to patient care in a wide range of clinical contexts.
5. Reflection:
Take a proactive and self-reflective role in evaluating and planning their continuing clinical and professional development as well as that of others within the practising team by engaging in feedback from a range of sources.

Syllabus

Assessment of patients:
- Obtain and record contemporaneously a comprehensive patient history under supervision.
- Undertake a systematic extra- and intra-oral examination with supervision.
- Undertake a dental development assessment of a child patient with supervision.
- Prescribe, carry out and interpret special investigations including dental radiographs, under close supervision.
- Develop skills in decision making with regards to diagnosis and treatment planning of common dental diseases (including dental pain) in the child and adult patient under close supervision.
- Engage with patients and parents undergoing orthodontic treatment.
Clinical operative management:
Undertake non-specialist care of patients in hospital (child and adult) and outreach clinics (adult only), under supervision. This will include:
- Safe practice within the clinical environment: including cross-infection control.
- Safe delivery of Dental local anaesthetic in adult and child patients.
- Non-surgical management of periodontal disease in adult patients.
- Conservative cavity preparation and restoration using a range of direct, plastic restorative materials in adult and child patients.
- Management of reversible and irreversible pulpal disease in adult patients*.
- Provision of removable prostheses in adult patients.
- Provision of extra-coronal restorations in adult patients*.
- A conformative approach to occlusal management.
- Simple tooth extraction of teeth and roots in adult and child patients under local anaesthetic*.
- Understanding the management of medical emergencies in the dental surgery. Basic life support skills.
- Management of pain due to common dental causes.
- Engaging with patients who are undertaking orthodontic treatment.
- Understanding management of the clinical orthodontic emergency.
Prevention:
Evidence based independent assessment of risk and the planning and provision of tailored preventive advice for common dental diseases in adult and child patients using best practice guidance. Awareness of the risks of smoking and alcohol on oral health and approaches to encourage patients to decrease these risks.
This will include:
- Oral hygiene analysis and advice
- Diet analysis and advice.
- Appropriate prescription and use of fluoride
- Smoking and alcohol assessment and advice.
Clinical Professionalism:
- Work with members of the dental team increasingly independent from support and understand how effective teams function to provide high quality patient care.
- Maintain currency in the knowledge and understanding of processes and guidelines within all clinical environments via engagement with mandatory training and local rules.
- Communicate effectively and sensitively with colleagues, patients and carers in relation to patient care, diagnosis, treatment planning and risks and benefits of treatment options with increasing independence. Some support may be required in challenging and difficult circumstances.
- Be able to facilitate gaining informed consent with limited support.
- Maintain good records in relation to patient care with limited support.
- Know and apply the core principles of confidentiality, professional ethics, raising concerns and act without discrimination against individuals or groups with limited support. Develop further understanding of these principles in the clinical setting and as applied to patient care.
- Act ethically with honesty, integrity, responsibility and respect for others.
- Practise leadership and management with a patient centred approach, demonstrating awareness, ownership and effective organisation and planning.
Reflection:
- Actively receive clinical feedback with independent practice of reflection.
- Actively engage in giving receiving peer feedback and in the patient evaluation process.
- Self-identify and record further development needs and demonstrate responsive action planning.

Opportunities for Formative Feedback

There are a number of monitoring and feedback opportunities throughout the module:
- Individual formative feedback is provided at each clinical practice session.
- The Clinical Assessment and Feedback System (CAFS) records clinical experience, student performance, staff and patient feedback and student reflection. Students can access this information via their individual analysis report.
- Monitoring and feedback on progress will take place during the clinical review process, this will involve students engaging in reflection and action planning.

Feedback is given on OSCE performance. Examiners give comments on performance in OSCE stations both generically and individually. Feedback comments are collated individually for students and are presented and reviewed in a whole year group session.

Due to the clinical nature of this module all summative and progressional assessments need to be passed independently. Students will present and defend a periodontology clinical case.
