Module summary

This module, the fifth in the ‘Clinical Practice’ modules thread, gives the opportunity for further clinical experience in a range of different speciality areas and in a range of clinical dental settings. The module runs throughout the entirety of year 5. The majority of the time will be spent providing care for patients within the Leeds Dental Institute (Restorative Dentistry, Oral Surgery, Oral Medicine Acute Dental Care, Paediatric Dentistry). You will also continue to gain experience of working in outreach clinics. You will gain more experience of working with assistants and be given the opportunity to assist for others. You will be expected to comply with all cross infection, health and safety and clinical governance protocols that are necessary for safe practice within the LDI as well as observing the relevant local practices in other settings. You will develop your ability to become a reflective practitioner and become aware of areas that need improvement, be well prepared for the procedures you are being expected to carry out and be proactive in seeking guidance and further development. You should be able to use tutor, peer and patient feedback effectively to reflect on your progress and to plan your ongoing development. You will also able to provide feedback to your peers and junior colleagues. You should be able to embrace the ideals of evidence-based dentistry and begin to take ownership of your own lifelong learning and continued professional development. Feedback will be provided by clinical tutors and progress will be monitored by the clinical progress committee. You will continue to use the CAFS system to record your clinical experience, feedback and reflection. Unless your programme is designed to be delivered fully online, your programme and modules will be delivered predominantly on campus. These teaching activities will be supported by the use of digital tools to enhance your learning and experience. The catalogue provides details of your module and the teaching methods used to deliver your module (for example, lecture, seminar, tutorial, practical). It does not necessarily provide you with information about the mode of delivery for specific learning activities on your module (whether these will be on campus or delivered digitally). Information about the delivery mode for learning and teaching activities for your modules will be published in the timetable and in the Minerva area for your modules. We sometimes need to adapt our teaching to take account of developments in the subject, in response to new research and current affairs for example, and on occasion we may need to adapt our delivery modes too. You will be kept informed of any proposed changes to the content and delivery of your modules as described in the module catalogue and we’ll make sure you understand and agree the reasons for any changes before they are introduced.

Objectives

This is the 5th module in the five-year clinical practice thread. In this module students will further develop their ability to take direct responsibility for the clinical management of dental patients.
The module objectives comprise the themes of:
- Assessment of patients
- Clinical operative management
- Prevention
- Clinical Professionalism
- Reflection
These themes are developed throughout the programme within the Clinical Practice (ICSP/CP 2-5) modules.

In this CP5 module there is opportunity to continue to observe and treat patients within the clinics of the Leeds Dental Institute and in outreach centres and observe specialist referral clinics in Oral and Maxillofacial Surgery and Oral Medicine.

Students will be encouraged and expected to continue to develop the knowledge and skills required to perform the safe, reflective and independent practise of dentistry to the level of the ‘Safe Beginner’. They should be able to provide a holistic dental treatment plan for adult and child patients who require dental care to a non-specialist level. They should be able to provide simple, non-surgical extractions, local anaesthetic procedures, plastic restorations, non-surgical periodontal treatment, removable prosthetic replacement of missing teeth, endodontic treatments, crowns and bridges and preventive care within a real life clinical environment.
The module will encourage continued development of a level of professionalism and independence commensurate with the increased level of responsibility of a graduating dental student. This will involve teamworking, communication and management and leadership including the ability to assist other clinicians, liaising with other specialists and being aware of their place in the dental team.
The module will further develop awareness of how the dental surgeon contributes to holistic, excellent patient care, as well as ensuring collaboration with other health care workers to achieve the best outcomes for the patient.
By the end of the module, students will be expected to have demonstrated their ability across all learning outcomes through a range of assessments:

- Formative (continued clinical assessment and feedback recorded in CAFS)
- Progressional (IRMER examination, Medical Emergencies Assessment, Progressional cases –Senior Operative Test, Prosthetics Case, Endodontics Case, Oral Surgery Case, simulated crown and endodontics exercises, Paediatric and Restorative Portfolio case presentations)
- Summative (Written examination, unseen diagnostic cases examination.)

The clinical progress of students will be monitored throughout the module by the School of Dentistry Clinical Progress Committee.

Learning outcomes
On completion of this module the students will be able to:
1. Assessment of patients:
Obtain, record and interpret a comprehensive patient assessment and assimilate clinical information to establish appropriate diagnoses and devise an evidence based treatment plan for their management in the context of holistic patient care, at the level of a ‘safe beginner’ as defined by the GDC.
2. Clinical operative management:
Carry out the full range of dental procedures in the context of whole patient care, with limited supervision, as required for a safe beginner in independent general practice.
3. Prevention:
Independently identify oral and general disease risk factors and co-ordinate management of a tailored, holistic oral and general health advice and preventive care, in patients with a range of clinical needs.
4. Clinical Professionalism:
Practise professionalism independently including team-working, communication, ethics and leadership to patient care in a wide range of clinical contexts.
5. Reflection:
Take a proactive and self-reflective role in evaluating and planning their continuing clinical and professional development as well as that of others within the practising team by engaging in feedback from a range of sources.
This module will develop knowledge, skills and professional attitudes to the level of the safe beginner in these areas.

Syllabus

Assessment of patients:
- Obtain and record contemporaneously, a comprehensive patient history.
- Undertake an accurate and systematic extra-oral, intra-oral and dental examination.
- Undertake a dental development assessment of a child patient.
- Prescribe, carry out and interpret special investigations including dental radiographs, independently.
- Further develop skills in decision making with regards diagnosis and treatment planning of common oral and dental diseases in the child and adult patient.
- Observe and gain experience of assessment of patients referred to Oral Surgery and Oral Medicine clinics.

Clinical operative management:
Undertake safe, non-specialist care of adult and child patients in hospital and outreach clinics (adult only), under reducing supervision.
- Safety of the clinical environment: including cross-infection control, safe instrument and sharps management, health and safety policy, reporting of incidents.
- Safe delivery of local anaesthetic in adult and child patients.
- Non-surgical management of periodontal disease in adult patients.
- Conservative cavity preparation and restoration using a range of direct, plastic restorative materials in adult and child patients.
- Management of reversible and irreversible pulpal disease in adult and child patients.
- Provision of removable prostheses in adult patients.
- Provision of extra-coronal restorations in adult patients.
- A conformative approach to occlusal management.
- Extraction of teeth and roots in adult and child patients under local anaesthetic.
- Understand and demonstrate (in the simulated environment) the management of medical emergencies in the dental surgery. Demonstrate skills in basic life support.
- Management of pain due to common dental causes

Prevention:
Evidence based independent assessment of risk and the planning and provision of tailored preventive advice for common dental diseases in adult and child patients using best practice guidance. Awareness of the risks of smoking and alcohol on oral health and approaches to encourage patients to decrease these risks.
This will include:
- Oral hygiene analysis and advice.
- Diet analysis and advice
- Appropriate use and prescription of fluoride
- Smoking and alcohol assessment and advice

Clinical Professionalism:
- Work with members of the dental team increasingly independent from support and understand how effective teams function to provide high quality patient care.
- Maintain currency in the knowledge and understanding of processes and guidelines within all clinical environments via engagement with mandatory training and local rules.
- Communicate effectively and sensitively with colleagues, patients and carers in relation to patient care, diagnosis, treatment planning and risks and benefits of treatment options with increasing independence at the level of the safe beginner.
- Be able to facilitate gaining informed consent independently.
- Maintain good records in relation to patient care independently.
- Know and apply the core principles of confidentiality, professional ethics, raising concerns and act without discrimination against individuals or groups with limited support. Develop further understanding of these principles in the clinical setting and as applied to patient care.
- Act ethically with honesty, integrity, responsibility and respect for others.
- Be aware of own limitations and know when to ask for help.
- Practise leadership and management with a patient centred approach, demonstrating awareness, ownership and effective organisation and planning.

Reflection:
- Actively receive clinical feedback with independent practice of reflection
- Actively engage in giving and receiving peer feedback.
- Actively engage in the patient evaluation process.
- Self-identify and record further development needs and demonstrate responsive action planning against nationally set capability statements for transition to the next stage of professional development.

Opportunities for Formative Feedback

There are a number of monitoring and feedback opportunities throughout the module:
- Individual formative feedback is provided at each clinical practice session.
- The Clinical Assessment and Feedback System (CAFS) records clinical experience, student performance, staff and patient feedback and student reflection. Students can access this information via their individual analysis report.
- Monitoring and feedback on progress will take place during the clinical review process. This will involve students engaging in reflection and action planning.
- Students will be given individual feedback on their progress following Sign up meetings 1,2,and 3.

Due to the clinical nature of this module all summative and progressional assessments need to be passed independently. The portfolio case and unseen cases examinations are clinical examinations and are held on campus within the Leeds Dental School and Leeds Dental Institute. Students will present and defend one Paediatric and two adult restorative clinical cases as part of their clinical portfolio exam.
