Module summary

This extended, in-depth clinical skills course provides both scientific knowledge and practical experience to prepare students to treat more advanced dental disease and its outcomes. Integration between dental disciplines, including Oral Surgery, conservative dentistry, endodontics and removable prosthetics.Basic sciences such as dental materials, anatomy and physiology will provide the undergraduate student with the necessary background to build their clinical decision making upon.A wide range of simulated clinical techniques are undertaken to restore badly broken down teeth, those with pulpal and peri-radicular disease, and the student is introduced to decision making processes leading to tooth loss and replacement. A strong thread of current evidence base and scientific understanding underpins each element of the course, together with an appreciation of changes in demands and expectations of our patients.Tooth removal (simple extractions) is introduced in a way which encourages students to consider the long term implications of tooth loss. The concept of 'planning for failure' and long-term prognosis for various treatment options is explored in some depth.

Objectives

- be able to make an appropriate pre-operative assessment of pulpal pathology;
- carry out orthograde endodontic procedures on teeth with multiple canals or curved canals, including re-treatment of failed endodontics;
- be able to safely use Nickle-Titanium and rotary instruments to achieve the aims of root canal therapy;
- be able to prescribe appropriate restorations for badly broken down teeth;
- be able to plan, design and carry out preparations for full veneer gold and metal ceramic crowns. Resin retained and conventional bridgework;
- be able to discuss the options for replacing missing teeth including shortened dental arch, implants, fixed and removable prosthodontics and orthodontics;
- be able to design appropriate removable dentures for partially dentate patients;
- be aware of the options for restoring root filled teeth and be able to place a direct post or amalgam dowel core;
- be able to carry out all porocedures listed with a knowledge of functional occlusion;
- be able to carry out simple tooth extractions and take appropriate measures if complications arise.

Learning outcomes
On completion of this course, students should be able to:
- diagnose and treatment plan for a range of more complex dental disease sequelae that may lead to advanced tooth or tooth tissue loss;
- consider and present a range of options for more complex restorations;
- plan and execute more complex restorative procedures in the clinical skills laboratory in harmony with the dynamic concepts of Occlusion;
- restore more badly damaged teeth satisfactorily, with good aesthetic outcomes;
- communicate effectively with the laboratory for the production of high quality complex restorations;
- integrate the concepts of pulp anatomy, physiology and pathology in relation to clinical disease presentation and treatment;
- manage the disease processes leading to loss of tooth vitality, and its outcome in terms of pathology and microbiology;
- be able to treat non-vital multi-rooted teeth using conventional endodontic therapy on extracted or simulated teeth;
- understand the mechanical and biological principles leading to successful treatment of the non-vital tooth;
- define and evaluate criteria for assessing the outcomes of vital and non-vital pulp therapy;
- consider the range of options for immediate and definitive tooth replacement;
- understand the concepts of minimal intervention dentistry, management of the residual dentition and replacement of missing teeth.

- Consent patients appropriately to undertake simple extractions in planned and acute clinical situations;
- undertake radiographic assessment of appropriate pre-extraction radiographs;
- plan and know how to provide definitive replacement options for a limited number of missing teeth, including fixed and removable options;
- have a working knowledge of laboratory procedures for fixed and removable options for tooth replacement;
- understand the limitations and longevity of the various treatment options and appreciate the need for assessment of long-term treatment needs.

Continuous assessment and progressional tutor sign off required for; Crown, endodontic (including case report), restoration and root filled tooth, occlusion, replacement of missing tooth-bridge and denture design courses. Basic extraction practical assessment. Students are required to demonstrate basic competence in extractions and have a level of confidence on a phantom head, before undertaking simple extractions when they start sessions on the Acute Dental Care clinic at the start of the Summer term. Resit opportunities will be provided to any student who fails the test either before or at the start of the Summer term. Students may not perform any element of a tooth extraction on a real patient until they have passed the test but they will still attend Acute Dental Care and work under close supervision. One attempt at the crowns test will be considered. Independent crowns test and coursework elements will provide summative 50% Exam and practical summative elements are non-compensatable
